The DUTCH Test: A Complete Guide to Understanding Your Hormones

By Mary Stratos, PA-C, IFMCP  ·  Hormonal Health

Your labs came back "normal," but you know something is off. The 3am waking, the mood swings that arrive out of nowhere, the energy that disappears by mid-afternoon. If a standard hormone panel has not explained what you are feeling, the DUTCH test often can.

What is the DUTCH test?

DUTCH stands for Dried Urine Test for Comprehensive Hormones. Instead of a single blood draw that captures one moment in time, you collect small dried urine samples at several points across a day, at home. From those samples, the lab measures not just your hormone levels but your hormone metabolites, the downstream products your body makes as it processes each hormone.

That distinction matters more than most people realize. Two women can have the same estradiol level on a blood test and completely different experiences in their bodies, because what your body does with a hormone is just as important as how much of it you have.

What it shows that standard labs miss

Estrogen metabolism. The DUTCH maps which pathways your body uses to process estrogen. Some pathways are protective, others are less favorable, and knowing your pattern helps personalize everything from nutrition to how we support detoxification.

Your cortisol rhythm. Rather than one morning cortisol value, DUTCH shows your pattern across the day, including the cortisol awakening response. This is where burnout, wired-but-tired evenings, and 3am waking often become visible on paper for the first time.

Progesterone and androgen metabolites. How your body processes progesterone influences sleep and mood. Androgen pathways help explain hormonal acne, hair changes, and libido, even when total testosterone looks fine.

Who it helps most

In my practice, DUTCH earns its place when the story and the standard labs do not add up: perimenopausal symptoms with "normal" blood work, fatigue that looks like burnout, stubborn hormonal acne, or fine-tuning for a woman who wants her plan built on the fullest possible picture. It is one of the most powerful tools in functional hormone medicine, and like any test, it is most valuable when someone interprets it in the context of your whole story.

What it is like to do

Simple, honestly. The kit comes to your home, you collect small samples over about a day, and everything mails back to the lab. No blood draw, no appointment, no time off work. Results take a few weeks, and then we sit down together and go through them line by line, in plain English.

How we use it at Stratos Functional Wellness

DUTCH is one of the advanced tests available in our care: included as one of the two case-matched tests in the Reclaim program, or available to Vitality Plan members when the clinical picture calls for it, always quoted before anything is ordered. The test is a tool. The medicine is the interpretation, and the plan we build from it.
